The Milo Care+ app provides Milo Care patients with access to all their vaccination records.
The App enables patients to create a SMART® Health Card which is a verified record of their COVID-19 test or vaccination.
The App also provides patients with a QR code to verify their CDC vaccination cards.
You may also request your child’s back to school Certificate of Immunization for school entry.

The SMART® Health Card was created by The Commons Project®. The Commons Project Foundation is a global tech nonprofit building solutions that empower people to access, manage, and share their data.
If you received at least 1 dose of vaccine from Milo Care then all doses you received in Florida will now be accessible to you in the Milo Care+ app.
New patients must download the app, login and then request a record upload from Milo Care support in the App or at www.milo.care
